5. Your Rights Regarding Your Health Information

You have the right to:

  • Access – Request to see or get a copy of your medical records.
  • Amend – Request corrections to your records if you believe they are inaccurate.
  • Restrictions – Request limits on certain uses or disclosures of your PHI.
  • Confidential Communications – Request that we contact you by specific methods or at specific locations.
  • Accounting of Disclosures – Request a list of certain disclosures we have made of your PHI.
  • Receive a Paper Copy – Request a paper copy of this Privacy Policy at any time.

6. Our Responsibilities

We are required by law to:

  • Maintain the privacy and security of your PHI
  • Provide you with this Privacy Policy
  • Notify you in case of a breach of your unsecured PHI
  • Follow the terms of this notice unless changes are made and shared with you in writing

We take appropriate administrative, technical, and physical safeguards to protect your information, including:

  • Secure electronic medical records system
  • Staff training on HIPAA compliance
  • Physical access controls to patient files
